📍 Location: Warren Gymnasium
Event Schedule:
• 8:00–8:15 AM – Information Session
• 8:15–8:30 AM – Summer Camp Sign-Ups
• 8:30 AM – Physicals Begin
Sports Physicals
• Available on site for $25 (cash or GoFan)
• Forms may be picked up in advance from the office, it is attached to this message, and can also be downloaded here: 👉 https://web.dusd.net/warren/athletics/
Important Physical Requirements:
• Must be completed by a doctor with signature and stamp
• Must be dated on or after June 8, 2026
• Valid for one year
• Families may also obtain physicals through their own physician.

Steps include:
• Create an account
• Select Warren High School and 2026–2027 school year
• Choose sport(s)
• Complete student and parent information
• Submit all required digital signatures

Important Reminder: Students must be fully cleared before participating in any after-school sport or activity.
