Please email the confidential referral form to TLCreferral@dusd.net .
Please consult with the Department of Children and Family Services at (800) 540-4000 in cases of suspected child abuse, neglect, and domestic violence.

Referrals can be submitted by DUSD faculty and staff ONLY.
Is it ok to have a parent call TLC directly without an actual TLC referral submission?
Can I call TLC to request support services for a family without completing a referral?
No, all official referrals must be completed and signed by an administrator.
How do I submit a TLC referral?

Must I make the parent aware that a referral will be submitted?
What if the student has or does not have health insurance?
